We want your visits with us to go as smoothly as possible.  In order for this to happen it is necessary for you to have a basic understanding of your insurance coverage.  Since we deal with approximately 800 different insurance plans, it is impossible for us to know the requirements of each patient’s individual plan.  We must rely on the patient for information regarding coverage, laboratory services, etc. 

Please use the following questions as a guide when contacting the customer service department of your insurance company.  Knowing the answers to these questions could prevent any unpleasant surprises when you receive your statement.

  1.       Is Dr. Swinyer a participating provider with your insurance plan?
  2.       What is your copayment?  (Copayments are due prior to your visit with the doctor.)
  3.      Do you have a deductible?  Has your deductible been met?
  4.       What labs may you use so your insurance will pay for laboratory services? 
  5.       What kind of prescription coverage do you have?
  6.       If you have two insurance plans, which one should be billed first?

**Please be aware that many dermatologic services are not medically necessary.  Medically unnecessary services will not be billed to insurance.
